Re: Consent to Release Confidential Information from Tenant

For the purposes of application in tenancy, under CASA P.O.W. Project, the

tenant hereby agrees to have their information released to agents

and associates of CASA P.O.W. Project. Such pieces of information may include, but not limited to:

rental history, employment or financial sources, medical or health problems,

reference names and numbers, civil status, citizenship or residency, utility bills and

whatever the client may provide willingly.

This information will continue to be used while the client is a tenant, but not after the tenancy ends.
